The Oregon Traffic Collision and Insurance Report (Summary)

Who Must File: Drivers must file a Collision & Insurance Report with the Oregon DMV within 72 hours if any of the following occur:
  • Damage to any vehicle exceeds $2,500.
  • Damage to any person's property exceeds $2,500.
  • Injury or death results from the collision.
  • Any vehicle is towed due to damage.
Even if a police report is filed, the driver is still responsible for submitting this report. Non-residents or out-of-state licensed drivers must also comply if the collision occurred in Oregon. *Failure to report may lead to suspension of driving privileges.

Instructions for Completing the Report:

  1. Complete both sides of the form in black or dark blue ink.
  2. Include details like:
    • Date, time, and location of the collision.
    • Driver and vehicle information, including insurance details.
    • Description of the incident.
  3. For collisions involving multiple vehicles, supplemental forms are required.

Reports can be submitted by fax, mail, or delivered to a DMV office. It’s important to keep a copy of the report and proof of submission since the DMV does not provide copies after submission

Special Instructions for Commercial Drivers:

Commercial motor vehicle operators must also file a Motor Carrier Collision Report within 30 days if:

  • There's a fatality.
  • An injury requires treatment away from the scene.
  • A vehicle is towed due to disabling damage.

Totaled Vehicle Reporting:

If a vehicle is considered totaled (damage equals at least 80% of its retail market value), owners must:

  • Surrender the title to the insurer or DMV.
  • Apply for a salvage title if they retain the vehicle.
  • Notify the DMV if a stolen vehicle isn't recovered within 30 days.

Failure to comply with totaled vehicle requirements can result in Class A misdemeanor charges.
